Simple subject+is/are/am+verb(4)+object object+is/are+being+past participle(V3)+ by+subject
Negative subject+is/are/am+ not+verb(4)+object object+is/are+not+being+past participle(V3)+by+subject
Interrogative is/are/am+subject+verb(4)+object is/are+object+being+past participle(V3)+by+subject
Interrogative+Negative is/are/am+subject+not+verb(4)+object is/are+object+not+being+past participle(V3)+by+subject

Active: He is playing football.

Passive: Football is being played by him.

 

Active: He is not playing football.

Passive: Football is not being played by him.
